As a courtesy to others, we reserve the right to reschedule your appointment if you are more than 10 minutes late. If you are unable to keep a scheduled appointment, please give 24 hours advance notice to ensure that you will not be charged for the appointment. If less that 24 hours’ notice is given, and we are unable to fill your time slot, your will be subject to a missed appointment fee of $25.

Copays are due prior to services rendered. Please present your Photo I.D. and insurance cards at the time of check in.
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.
